They reworked it in May of 05---

So now, not only does it list all of the restaurants, prices, space for ADR#s, but it also lists where the characters appear so that you can plan your meals around them. It gives you space to plan for the day's activities and it takes the DYW in consideration. It has spaces for your other vacation related costs such as plane ticket costs and more.

It is an exel download, takes about 2 seconds to do.
I love this form and have started filling it in for Nov. 06
