I plan on going on a trip to WDW in a month or two, and I'll be taking along a 16-year-old and a 17-year-old (I'm 18). Do I need to get their parents to sign any forms or documents or such for like First Aid, Proof that they're allowed to come with me, etc.?

I don't think that having them sign a consent form for medical attention would be a bad idea. It surely can't hurt. I am not certain of the laws with this. I would definately have their medical insurance cards and permission slips for medical attention just in case the worse should happen.

I would also get the parental consent forms, just in case. I'm not sure if there are age limits of treating "kids" without the parents there, but it's better to safe. And yes, as Dana said, be sure they carry their insurance card too. It's unlikely that you'll need it, but you never know.

I've made them up myself when I take other people's kids with me. In case the child needs any medical attention, you'll need it. And my dad's a notary, so I get it signed by him too. Include the child's full name, address, DOB, the dates you will be traveling with them in your care, and medical insurance info. Probably should have the child's pediatrician's number there also. End it with stating the parent gives you temporary guardianship for those dates in case of an emergency.

Oh yeah. Any allergies and/or medical conditions should be there too.
You don't need to pay for a form to print that info out.

You just need to add some medical info on there. Hospitals and walk-ins want something for billing purposes, no matter how urgent. Many teens have their own copies of medical cards, depending on the carrier.

I take a group of teenagers every year. We require a notorized form with their parent's signature. If you want, PM me and I will email you a copy of it. We also require the parent's to give us permission to provide their teenager with any medication (aspirin, etc.) Of course, I am leading a school group, so we really protect ourselves. They should also have an insurance card with them if they have insurance.
Also, one CM when I was making reservations faxed me a travel indeminety (spelling is not correct) form that said I had permission to bring someone that it was not my child and was underage. This form basically released Disney from any responsibility medically or legally. But, since we always go through youth groups, I have not had to use that form.

Even with the notarized consent, the paramedics still talked to his gaurdian. Better to be over prepared and not need it.
